Immediate placement and loading

Out, in, and
a tooth the same day.

In the right site, the tooth comes out, the implant goes in and a temporary goes on within a single appointment. The skill is not in doing it. It is in knowing which sites allow it.

Why people come here

  • Surgery and the finished teeth done by one dentist, under one roof
  • Diplomate of the International Congress of Oral Implantologists
  • Fellow of the Misch Institute — and faculty at its successor
  • A written, itemized plan before anything is scheduled

Two separate decisions

Placing it and loading it are not the same thing.

Immediate placement means the implant goes into the socket at the same visit the tooth is removed. Immediate loading means a temporary tooth goes on it straight away. They are often discussed as one thing and they are not.

You can place immediately and let it heal quietly under the gum. You can also place and load, but only when the implant is stable enough at the moment of placement to tolerate it. That stability is measured, not guessed.

What has to be true

  • Intact socket walls — particularly the thin one on the cheek side
  • No active infection at the site
  • Enough bone beyond the socket to get primary stability
  • A bite that can be adjusted so the temporary is not being hammered
  • A patient willing to stay on a soft diet through the early healing
SAME VISIT · EXTRACT, PLACE, PROVISIONALISE
Immediate placement with a provisional the same visit

The honest trade

What you gain, and what you risk.

The gain

Months saved, one surgery instead of two, and a ridge that is supported from day one rather than left to collapse and be rebuilt later.

The risk

An implant loaded before it is ready is an implant that can fail, and a failed immediate case usually costs more time than the staged route would have taken.

Questions

Same-day, answered.

Is the same-day tooth the final tooth?

No. It is a provisional — shaped to guide the gum and let you function, but made to be replaced. The final restoration is made after integration, when the tissue has settled into its finished shape.

What can I eat?

Soft food for the first several weeks, and nothing that asks the temporary to do heavy work. This is the part patients underestimate and it genuinely affects the outcome.

If my site does not qualify, what then?

The socket is grafted at the time of extraction and the implant is placed a few months later into rebuilt bone. It is slower, it is highly predictable, and it is frequently the right answer.

Start with an exam and a straight answer.

A consultation includes an examination, 3D imaging and a written, itemized plan. If implants are not the right treatment for you, Dr. Patellis will say so and tell you what is.
